Add terms hereIn consideration of being permitted by College Golf Experience, LLC (“CGX”) to participate in College Golf Experience Programs and Events (“CGX Programs”), now and in the future, I, (“Parent or Legal Guardian”) of my minor child/ward (“Student”), hereby grant CGX permission to allow Student to participate in CGX Programs under the terms of this Agreement.

1. Assumption of Risk. Releasing Parties (as defined in Section 2 below) hereby accept and assume all risks inherent in Student’s participation in CGX Programs. Releasing Parties acknowledge and agree that the activities that Student will voluntarily engage in include certain known and unanticipated risks which could result in harm to Student, property and/or third parties. Releasing Parties acknowledge and agree that such risks cannot be eliminated without jeopardizing the nature of the activities included in CGX Programs.

2. Release; Covenant Not to Sue; Indemnification. I, on my behalf, and on behalf of Student, my spouse, children, parents, heirs, successors and assigns, administrators and personal representative (“Releasing Parties”), hereby release and forever discharge CGX, its agents, owners, directors, managers, members, partners, employees, affiliates, subsidiaries, related and affiliated entities, successors and assigns, administrators, partners, sponsors, advertisers and owners/lessors of the premises used to conduct CGX Programs (the “Released Parties”), agree not to sue Released Parties and to indemnify, defend and hold Released Parties harmless from any and all claims, demands, costs, expenses, damages and causes of action arising from Student’s participation in CGX Programs and/or use of CGX equipment, to the fullest extent allowed by law.

3. Medical Authorization; Health Representation. If Student becomes ill or injured or requires medical assistance while participating in CGX Programs, I assume liability for and will pay all related medical expenses. I acknowledge and agree that Student’s participation in CGX Programs is purely voluntary and elect to have Student participate in CGX Programs despite all risks, known or unknown, including, without limitation, exposure to contagious diseases. I certify that, to the best of my knowledge, Student does not currently have any health condition that would make it inadvisable for Student to participate in CGX Programs.

I grant CGX, its affiliates, partners, and sponsors, the irrevocable right and permission to photograph and/or record Student’s and Parent or Legal Guardian’s participation in CGX Programs and to use such photographs and recordings for advertising and promotional purposes, in perpetuity throughout the world, and without restriction as to alteration. I waive any right to inspect or approve the use of such photographs and/or recordings, and acknowledge and agree that the rights granted to CGX in this Section 4 are without any expectation of compensation of any kind. All photographs and/or recordings will be the property of CGX.

I grant CGX, its affiliates, partners, and sponsors, the irrevocable right and permission to use the information provided during registration about the Student and Parent or Legal Guardian. This information will be used at CGX Programs and Events, will be shared with coaches, and CGX partners and sponsors for their use, will be used for CGX’s advertising and promotional purposes, and in CGX’s app(s), in perpetuity throughout the world, and without restriction as to alteration. I waive any right to inspect or approve the use of such information, and acknowledge and agree that the rights granted to CGX in this Section 5 are without any expectation of compensation of any kind.

I acknowledge and agree that I have had sufficient opportunity to read this Agreement in its entirety, understand its content and understand that it affects my legal rights and the legal rights of the Releasing Parties, and agree to be bound by its terms.
